Permission to Participate - Please read and select accept – registration forms cannot be accepted without accepting. The child listed on this form have my permission to participate in this group. PLEASE PROVIDE ANY MEDICAL ASSISTANCE NEEDED WHILE THIS PERSON IS WITH THE GROUP. I, the natural parent or legal guardian, do hereby authorize Saginaw United Methodist Church (SUMC), a member of the Central Texas Annual Conference, and it’s delegated leaders and staff as agent(s) for the undersigned, to consent to any medical and hospital care which is deemed advisable by and is to be rendered under the general or special supervision of any licenses physician or surgeon or the medical staff of a hospital, whether such diagnosis or treatment is rendered at the office of said physician or at said hospital. It is understood that this authorization is given in advance of any specific diagnosis, treatment, or hospital care being required, but is given to provide authority and power on the part of the aforesaid agent(s) to give specific consent to any and all such diagnosis, treatment, or hospital care deemed advisable. This authorization shall remain effective while the person named above is under the care and custody of SUMC. Excepting limitations stated herein the person registered on this form has permission to participate in all program activities under the supervision of the church staff and its designated leaders. I, the undersigned, acting on behalf and that of the above named minor(s), do herby waive all claims against and agree to indemnify and hold harmless Saginaw United Methodist Church, a member of the Central Texas Annual Conference, its officers, staff and designated leaders from any liability or responsibility for any injuries received by myself or my child (children) while participating in this church program(s.) I, on my own behalf and on behalf of my Child, herby warrant that I have read this release in its entirety and fully understand its contents, and am aware that this form releases the church of SUMC from liability, and have signed this form of my own free will. I understand that authorization shall be effective continuously from the date hereof until canceled by written notice to the church. I agree to update this information in writing as the need arises. I understand that should my child become a disruptive force during the program that the Director may choose to release him/her from the program and they will need to go home without refund.

Media Release - I give permission for media such as photography, video, artwork, and/or sound clips of my child to be shown in worship at Saginaw united Methodist Church and to be uploaded to the SUMC website, Vimeo, and Facebook page, and other forms of media noting that first names only will be used. I further waive, release, absolve and agree to indemnify and Saginaw United Methodist Church harmless as a result of participation in any programs sponsored by the Church.
